The basic outline of this form is really useful, however I wouldn't be caught dead carrying that actual one-sheeter. It's a good example of the kind of things you need to put on a page like that. The article itself is unspecific and misguiding. Not in a million years would I ever print a sheet that had a heading "This is why I rock," or "Brilliant ideas." And while I understand that this is not exactly what the article is trying to get across, I'm just embarrassed reading it and really hope that people aren't printing that chart out with them and carrying it around to interviews. I would be mortified if a potential future employer caught a glimpse of that.
This template is a great idea to study and remember simple questions and more specifics about yourself so you can recall them on a fly. I agree with Abigail, I definitely would not have this out while I was in an interview, But I could use this template to help my recall when I'm on the spot. Hopefully individuals will put this sheet away before they go into an interview. Although it shows you are prepared, there are ways to make it more professional. Adapting some of the sections like "This is why I rock" might be better.
I like the idea of going through the most commonly asked interview questions and having your personal answers well thought out. In an actual interview I don't want to come off as memorizing an answer. I want my true feelings at the time to come through so I seem sincere. Preparing for interview this way will help you think so when the time comes, you aren't floundering for words. I would not, however, enter an interview with my answer sheet in front of me. I believe in having a one on one and eye to eye conversation where I can speak from my heart and not have to look at a piece of paper. Coming off as confident is important in any interview so being prepared is important but having to depend on a paper in front of you is not my style.
